We should be grateful if you would complete the attached form in order that we can complete The Early Years Census 2009 and Spring Term Headcount being run by Wilts County Council.

Some of you may receive forms which are pre-printed, please check the information and amend where necessary, signing the form at the bottom. Please return this to us at soon as you can.

For those of your who do not have a pre-printed form, please will you print it out and return to us as soon as you can.
